The Chain Breakers Project Intake Process
Getting started with the Chain Breakers Project is simple, supportive, and tailored to meet your individual needs. Once you express interest, you’ll receive a personalized email from one of our qualified support specialists who will schedule your official intake appointment. We are currently working out of Jonahville AME Zion Church, where you can book your appointment online or visit us in person to begin the process.
During your intake, we’ll take the time to understand your goals, challenges, and strengths. Together, we’ll determine a pathway for success—whether that means completing your HiSET, exploring career options, improving life skills, or accessing community resources. Our team includes professionals who specialize in evaluating IEPs, 504 Plans, assisting with job placement, and providing elderly support services.
